Why does the county matter when you look for senior care in West Michigan?

The county decides which aging-services office and MI Choice agents you call, and the ZIP code decides which PACE program can enroll you. Ottawa County is the outlier: it belongs to Region 14, served from Muskegon.

Licensing does not change across county lines -- LARA applies the same home for the aged and adult foster care rules statewide -- but the people who help you apply for Medicaid programs do. A family with a parent in Jenison and a sibling in Kentwood is dealing with two regional agencies even though the homes are ten minutes apart. Each county page lists the right office, the PACE coverage by ZIP, and every licensed home in the county by town.

Hospital systems cross county lines freely: Corewell Health runs hospitals in Grand Rapids, Zeeland and Greenville; Trinity Health in Grand Rapids and Grand Haven. Discharge planners at any of them can refer to homes in all five counties.

Which county has the most licensed senior care?

Kent County, with 199 of the 343 LARA-licensed homes we track and 27 of the 49 nursing homes.

Ottawa follows, concentrated in Holland, Grand Haven and the Georgetown Township suburbs. Allegan, Montcalm and Ionia have fewer homes, spread across small towns, which is why families there often search across county lines.

Do residents have to choose care in their own county?

No. Families can choose a licensed home anywhere; only program enrollment -- MI Choice region, PACE ZIP code, Medicaid case office -- follows where the person lives.

A move across a county line can change which MI Choice agent or PACE program serves your parent, so ask before moving.

Which hospitals anchor each county?

Kent has the Grand Rapids hospital systems -- Corewell Health, Trinity Health Grand Rapids, University of Michigan Health-West and Mary Free Bed; Ottawa has Holland Hospital, Corewell Health Zeeland and Trinity Health Grand Haven; Allegan, Montcalm and Ionia each have a community hospital.

Hospital discharges drive many placements, so the nearest system often shapes which homes a family sees first.
