Why do so many Grand Rapids homes rate either five stars or one?

Because two kinds of homes share the Kent County list: hospital-affiliated rehab units and continuing-care campuses, which tend to staff heavily and rate high, and large standalone long-term care homes, several of which have struggled on inspections.

The overall rating starts from the health inspection rating, then moves up or down for staffing and quality measures. A home with many serious citations starts low and is hard to lift. That is why a family comparing a short-stay rehab unit with a long-term home is comparing two different businesses.

Which West Michigan nursing homes carry federal warning flags?

Six do: two are listed as Special Focus Facility candidates and five show the Care Compare abuse icon, with one home carrying both.

CMS uses the Special Focus Facility program for homes with a persistent record of serious problems, and shows the abuse icon after abuse-related citations. Neither flag tells you what happened last month; both tell you to read the inspection report and ask questions.

What should you read beyond the star rating?

Read the citation list from the latest inspection, the staffing hours, and the turnover numbers; together they explain the star.

Each citation has a federal tag and a scope-and-severity letter from A to L; letters G and above mean actual harm or immediate jeopardy. Staffing hours per resident per day vary more than twofold across West Michigan homes. High nurse turnover often shows up in the inspection record a year later. Our nursing home directory shows each home's stars, last inspection and deficiency count.

Does a five-star rating guarantee good care?

No. Ratings are built from inspections, reported staffing and quality measures over a period of time, and they lag change -- a new administrator, an ownership change or a staffing crisis can move a home before the stars do.

Visit more than once, at different times. Ask how many aides work evenings and weekends. Ask whether the home has changed ownership recently -- CMS flags changes within the last 12 months.

How do you use the ratings for a rehab stay after a hospital discharge?

Start with the homes that accepted the hospital's referral, then rank them by health inspection and staffing stars, and ask each about therapy minutes and the physician who will see your parent.

Short-stay quality measures, such as how often residents improve in function or are readmitted to the hospital, are on each home's Care Compare page and matter more for rehab than the long-stay measures.
