How does PACE decide who lives in its service area?
By ZIP code. MDHHS publishes each PACE organization's approved area as a list of counties and ZIP codes, and living inside that area is one of the eligibility requirements, along with being 55 or older, meeting Medicaid's long-term care criteria and being able to live safely in the community.
That makes a parent's address the first thing to check -- before diagnoses, before finances. A move of a few miles can put someone inside or outside a program.
Which PACE programs cover West Michigan, and how much of it?
Six programs reach into our five counties, with very different footprints.
| Program | Where it reaches in our area | Phone |
|---|---|---|
| Care Resources (Grand Rapids, Lowell) | All of Kent County; listed ZIPs in Ottawa, Allegan and Ionia | 616-913-2006 |
| LifeCircles-PACE (Holland, Muskegon) | Most Ottawa ZIPs; Lakeshore and western Allegan ZIPs | 616-582-3100 |
| Senior Care Partners PACE | Plainwell, Otsego and other southern Allegan ZIPs | 517-680-4410 |
| PACE Central Michigan | Greenville, Carson City, Crystal, Edmore (Montcalm) | 989-953-5800 |
| Community at Home PACE | Howard City, Coral, Trufant and other northern Montcalm ZIPs | 800-689-6675 |
| VOANS Senior Community Care of Michigan | Portland (Ionia County) | 517-319-0700 |
ZIP lists are from MDHHS's PACE provider page as of October 2026.
Which West Michigan towns fall in two PACE areas?
Byron Center (49315) appears on both the Care Resources and LifeCircles-PACE lists, and the city of Allegan (49010) on both the LifeCircles-PACE and Senior Care Partners lists.
For families in those ZIP codes, the choice comes down to which day center is easier to reach and which program's doctors and hospital relationships fit. Call both and ask about transportation from your parent's address.
Which towns have no PACE coverage?
Of the towns on this site, Lakeview (48850) in northern Montcalm County is the one whose ZIP code does not appear on any published PACE list.
For Lakeview residents, MI Choice is the main Medicaid route to nursing-home-level care at home; the Area Agency on Aging of Western Michigan in Grand Rapids handles intake for Montcalm County at 616-456-5664.
Why does Jenison use a different PACE program than Hudsonville?
Because Care Resources' list reaches into Ottawa County only for Jenison (49428), the Grandville ZIP (49418), Marne (49435) and the Standale-area ZIPs (49534 and 49544), while Hudsonville (49426) is on the LifeCircles-PACE list.
Neighboring suburbs can land in different programs. If a family is choosing between homes on either side of that line, the PACE program -- and its day center -- may be a reason to prefer one.
What should a family do once they know the ZIP is covered?
Call the program for an intake conversation; it will screen for nursing-home level of care, help with the Medicaid application and explain what changes, including switching to PACE's own doctors.
Remember that PACE and MI Choice cannot be combined. If a parent is already in MI Choice, moving to PACE means leaving the waiver. Our PACE page lists every program's centers and how each town on this site lines up.

Yes in most ZIP codes: LifeCircles-PACE covers much of the county, and Care Resources covers Jenison, the Grandville ZIP, Marne and the Standale area.
